In all of the locations that Synthesia searches for MIDI files, if the there are any MIDI files with the exact same contents [the same MD5 hash], then Synthesia will only show the first file it found from each set of identical files.
On Windows, you can find duplicate files by using the NirSoft utility SearchMyFiles. In the first drop down box Search Mode, select "Duplicates Search". In the search results, you can click on the column header "Duplicate Number" to group the files in sets of identical files.

Just an additional note: because this behavior of weeding out duplicates in the song list seems to cause more confusion than anything else, it's on the to-do list to remove it and simply show all files in all folders all the time.
